Cash-back Claim Form

  1. Please complete and submit the following Cash-back Claim Form.
  2. The Cash-back Benefit entitles you to a rebate of 25% on the cost of all merchandise purchased by you from The Unlimited Play.
  3. Once a month The Unlimited Play will calculate:
    i. the value of all your Unlimited Play purchases (online, call centre); and
    ii. the Cash-back Benefit due to you.
  4. The Cash-back Benefit will be paid to you in accordance with the cash-back terms and conditions.
  5. The Cash-back Benefit cannot be used in conjunction with any other Unlimited Play vouchers, discounts or promotions.
